1. I acknowledge that I am at least 16 years old and have the legal capacity to enter into this agreement.
2. I agree to follow the rules and policies of Thomas elementary and its staff, and to conduct myself in a professional and respectful manner while on school property or participating in school-sponsored activities.
3. I understand that I will be working with children and youth, and that I must maintain confidentiality and respect their privacy.
4. I agree to provide accurate and complete information on my application, including any medical or physical limitations that may affect my ability to participate in volunteering.
5. I understand that I am not entitled to any compensation or benefits for my volunteering services but may be eligible for recognition or rewards as designated by the school.
6. I agree to hold harmless and indemnify Thomas elementary and its employees, agents, and volunteers against any claims, damages, or expenses arising from any injury or loss caused by my actions or omissions while volunteering.
7. I understand that this agreement shall be governed by and construed in accordance with the laws of California.
2024 Liability: I understand that volunteering at Thomas Elementary Community Schools may involve various activities, including lifting, moving equipment, and handling materials. I hereby accept and assume full responsibility for any injury, loss or liability I might incur while volunteering at Thomas Elementary Community Schools. In the event of an injury, parents/guardians authorize school staff to seek treatment for minor volunteers and to take other action should a medical emergency arise. I waive and release my right for damages.
2024 Photo Release: By participating in school activities, I hereby grant permission to Thomas Elementary Community Schools to use my image or likeness for promotional purposes, including but not limited to, school publications, social media, and other marketing materials. Thomas Elementary Community Schools has the right to edit or modify these images as needed.
